• Fill out the form. Please ensure information is accurate and up to date.

• Submit your information.

Our Review Process

• Once submitted, the Breastfeeding Hawaiʻi team will review your request to ensure it aligns with our mission and community needs.

Notification of Decision

• You will receive an email response within 7-14 business days confirming approval, requesting edits, or notifying you if the request cannot be approved.

Updates & Changes

• Approved vendors may log in at any time to request updates to their information.
